99% percent of the times, the movies do not feature real sex.
Why would they? Real sex in mainstream movies is messy and very difficult to control, rehearse and act.
The sex scenes in movies aren’t actually real. We just see actors pretending to take pleasure in those activities. But there are exceptions.
There have been a handful of movies that have taken it to the extreme. These movies weren’t pornographic, but still, the sex scenes that they consisted of, were real.

1. All About Anna
All About Anna tells the story of Anna (Gry Bay).
She is a single woman who seeks to maintain an active sex life while staying clear of emotional involvement, after having been jilted by the love of her life, Johan (Mark Stevens).
She has a relationship with Frank (Thomas Raft), but refuses to let him move in with her. When Johan shows up again after five years absence, she starts wondering how much longer she can maintain her emotional independence, and if that is what she wants.
2. 9 Songs
9 Songs is a 2004 British art romantic drama film produced, written and directed by Michael Winterbottom.
The title refers to the nine songs played by eight different rock bands that complement the story of the film.
It was controversial upon original release due to its sexual content, which included unsimulated footage of the two leads, Kieran O’Brien and Margo Stilley, having sexual intercourse and performing oral sex as well as a scene of ejaculation.
The film was showcased at the Cannes Film Festival.
3. Wild Orchid
Young lawyer Emily Reed (Otis) travels to New York City for an interview with an international law firm.
This firm immediately offers her a job on the condition that she can fly to Rio de Janeiro the following morning. Emily agrees and is introduced to Claudia Dennis (Bisset), one of the firm’s top executives.
They arrive in Rio to finalize the purchase of a hotel, but angry Claudia must fly to Buenos Aires, Argentina, to meet the hotel’s owner.
Claudia instructs Emily to cover her date for the night. While viewing the hotel, Emily sees two locals having animalistic sex, which unnerves her and she returns to her own hotel.
She meets Claudia’s date; a wealthy man named James Wheeler (Rourke). They have dinner, accompanied by James’ bodyguards.
